There are three general options available to you for breast reconstruction:

Do I need to have breast reconstruction?

No, breast reconstruction is not mandatory or necessary. Some women decide that they are not willing to have breast reconstruction or may not want to undergo any more surgeries. Some patients prefer to wear artificial breasts called a prosthesis. The goal is to retain femininity after a mastectomy.

How long is the breast reconstruction process?

The length of the procedure and the number of surgeries depend on various factors. One of those factors is your cancer treatments and whether they are ongoing or not. Usually, the breast reconstruction procedure takes six months to a year if there is no need for more cancer treatments. It also depends on the patient’s preferences. They could want the reconstructed breast to match their natural breast, and that could take longer.

Are there risks associated with breast reconstruction?

There are risks associated with any surgery, and the same is true for breast reconstruction. Dr. Tutino will discuss these with you in detail when you visit us for your initial consultation. He will also answer all of your questions satisfactorily. Some risks associated with breast reconstruction are:

What if my natural breast does not match my reconstructed breast?

Dr. Tutino’s ultimate goal for breast reconstruction is to create a breast that looks the same as the remaining natural breast, and both are symmetrical. Sometimes, it is easily achievable. Other times, matching the two breasts becomes difficult unless surgery is done on the remaining natural breast. It might mean using a breast implant on the natural breast or making it smaller through breast reduction surgery. It could also involve breast lift surgery to lift it. Dr. Tutino will discuss these issues with you in great detail during your consultation. What if I need further chemotherapy?

Chemotherapy treatments are usually not affected by breast reconstruction. But it would be best if you still discussed it with your oncologist. They will usually wait until you are done with your mastectomy and breast reconstruction before starting your chemotherapy. They could delay the treatment further if you have open wounds or infections. 

What if I need radiation?

Typically, it is best to complete your radiation therapy before starting with breast reconstruction. It could affect the outcome if you start breast reconstruction while having radiation therapy or starting radiation therapy during breast reconstruction.

When Should You Have Breast Reconstruction?

Some women want to have breast reconstruction as soon as possible, while others choose to wait months or years after finishing their breast cancer treatment. In most cases, if breast reconstruction does not interfere with any existing medical condition, patients can proceed immediately.

Sometimes, a waiting period is recommended or required. For instance, after chemotherapy, you might have to wait for 3-4 weeks, and after radiation, you might wait for 4-6 weeks. It could be longer depending on your condition and the condition of the tissue in the affected area, which is imperative because your body needs time to heal. Dr. Tutino will ask questions about your treatment history and overall health before offering you specific, honest feedback.

After Breast Reconstruction

n important aspect of planning breast reconstruction surgery is a frank discussion between the patient and plastic surgeon regarding expectations. The truth is that breast reconstruction does not entirely restore a person’s original natural look, feel, or sensation in most cases. After full mastectomy, flap restoration and areola reconstruction, new breasts are created from existing tissue and implants. That means they may look fabulous, but they are not expected to replicate your original breasts.

Nerves found deep in breast tissue provide feeling to the nipples, so with major nerve and tissue removal, reconstructed breasts and nipples don’t usually “feel” the same.

It’s also essential to understand that scarring may be inevitable after your breast reconstruction. In many cases, Dr. Tutino can revise or improve the look of existing breast cancer scars. However, any surgical procedure requires new incisions. The extent and placement of yours will depend on your unique case. Scars are expected to fade and soften over several months. Dr. Tutino will advise you about scare care or treatments that can help minimize their appearance.

The recovery process after breast reconstruction could take several months. Dr. Tutino will tell you when you may return to your normal routine and when you can start exercising.


Nipple And Areola Reconstruction

If you have had a nipple-sparing mastectomy, then the nipple and the areola will still be in place after the surgery. Otherwise, you could choose nipple and areola reconstruction surgery. This procedure is usually done after the reconstructed breast mound heals and settles in; it usually takes 3-6 months.

Types Of Breast Reconstruction

Breast Implants

Breast implants are tear-shaped or round, silicone gel or saline (water) filled. Today’s newest generation of breast implants is available in diverse size ranges to suit all body types and anatomy, including asymmetrical breasts.

Tissue expansion is used to create more space when tight chest skin does not accommodate a desired breast implant volume. Dr. Tutino will place an expander implant, a balloon device with a valve, under the skin, breast, and muscle in the intended implant location. Saline is injected into the balloon using a small needle; then, gradually over several weeks, the size may be increased. Once there is enough space, Dr. Tutino takes out the expander implant and places the permanent implant.

Dr. Tutino also performs one-step reconstruction or direct-to-implant reconstruction using acellular dermal matrix material to help strengthen skin covering implants for a strong, natural-looking contour. Strips of the acellular dermal matrix are placed under the breast skin to create a pocket for the implant. Over time, the strips grow into the resilient breast tissue.

Concerns About Breast Implants

Many people have concerns about silicone breast implants and their potential effects on a person’s health. According to multiple studies, today’s Health Canada and FDA-approved silicone breast implants are safe and don’t increase health problems. Rarely, a condition can develop called non-Hodgkin lymphoma. It is also called breast implant-associated anaplastic large cell lymphoma (BIA-ALCL). The starting point of this condition is typically traced to scar tissue formed around the implant. According to recent research, women who choose textured implants have been more at risk of this condition than those with smooth implants. More research is underway. If you have any questions or misgivings about breast implants, feel free to discuss them with Dr. Tutino during your initial consultation.

Removing Breast Implants

Breast implants, like any medical device, are not guaranteed to last indefinitely. You can also choose to have them removed any time you want, for any reason. Many breast augmentation patients wait 15 – 20 years or more before having their implants removed or replaced.

Considerations for Breast Reconstruction

There are risks and considerations for breast reconstruction. These depend on the type of surgery your doctor recommends and your condition. If you have undergone radiation, you and your plastic surgeon might be watching more closely for certain complications than someone who hasn’t been treated with radiation.

Most expected, inflammation and bruising will resolve within a couple of weeks of surgery. Other issues are less common, unexpected, or longer-lasting.

These include:

  • Infection
  • Delayed wound healing
  • Breast Reduction
  • A buildup of fluid (called a seroma) in the breast
  • Persistent pain
  • Scar tissue around the breast implant (called capsular contracture), which makes the breast feel hard and change shape
  • Malposition of the implant
  • Rippling of the implant, which makes the skin on the breast appear rippled
  • Leaking or rupture of the implant
