BRCA Breast Cancer Testing 

Many women who have a known history of breast cancer diagnosis in their families will undergo genetic testing through their general family physician. Genetic testing identifies mutations in genes, including BRCA1 and BRCA2. These are considered to be strong indicators of increased breast or other cancer risks. Women of Ashkenazi Jewish descent show a higher BRCA1 and BRCA2 mutation prevalence. 

It is important to note that genetic testing doesn’t indicate a cancer diagnosis or guarantee of breast cancer development. What Is Prophylactic Mastectomy? 

also known as a preventative mastectomy, or BRCA mastectomy, this breast surgery is performed for those patients who test positive for certain genetic factors that indicate high breast cancer risks. 

Mastectomy techniques have changed dramatically throughout recent years. Radical mastectomies performed in the 1950s, '60s, and '70s involved total removal of breast tissue along with breast skin, nipples, lymph nodes, and muscle. Starting in the 1980s, more selective, modified mastectomies left chest muscles intact. Next, the evolution of skin-sparing mastectomies focused on preserving breast skin while removing glandular and fat tissue so that reconstructive, restorative procedures could be done.

Understandably, total removal of breasts can be a life and confidence-altering step for women who commit to dramatic figure changes and a lifetime of wearing prosthetic bra inserts to fit clothing, feel feminine and proportionate. Though in many cases it was lifesaving, the negative mental and emotional aspects of previous mastectomy methods left much room for improvement.  

Nipple-sparing mastectomies leave nipple, areola, and breast skin envelope intact. This technique has gained traction since the early 2000s and has become a preferred option for patients with breast cancer or a high risk of breast cancer development.

Typical breast reconstructive surgery involves the mastectomy performed by a general surgeon, then after a recovery period, reconstruction can be performed by a plastic surgeon. 

Mastectomy techniques include: 

  • Total (simple) mastectomy
    A total mastectomy removes the whole breast, including the nipple-areola and a section of skin. This typically results in a horizontal scar in place of each breast. 
  • Modified radical mastectomy
    The entire breast including areolas and breast skin is removed. Partial removal of chest muscles and some lymph nodes may be included.

  • Radical mastectomy
    The entire breast is removed, including axillary lymph nodes and pectoral chest muscles. Today this drastic excision is rarely required except in cases where breast cancer spread warrants it.
  • Skin-sparing mastectomy
    This technique removes breast tissue and areolas but preserves most of the breast skin. The skin-sparing double mastectomy method works well to facilitate immediate reconstruction of the breasts. A new nipple-areola complex can be created with skilled surgical techniques and tattoo effects complete natural-looking results. 
  • Nipple-sparing mastectomy
    Many women ask, “do you keep your nipples after a mastectomy?” Thankfully, today’s skin-sparing mastectomy method allows women who qualify to keep their natural nipples, making reconstruction look and feel the most natural. For those who choose mastectomy as a preventative measure rather than cancer treatment, nipple-sparing mastectomy is often possible. All breast tissue is removed but the nipple and areola are preserved.
